Refresher on Your Referral Funnel: Are You Easy to Refer To?

Referrals are the lifeblood of your LTC@Home pharmacy model—but many pharmacies haven’t reviewed their referral process in months. June is a great time to step into your referral partner’s shoes and ask: How easy are we to work with?

Do partners know where to send referrals? Are forms simple and accessible? Do they receive a confirmation when a patient is onboarded?

Make your process patient-centered and transparent. Offer referral packets to physician offices, include instructions on your website, and consider using a digital referral form that integrates with your workflow.

Also, be proactive with communication. Call or email partners with updates once their patient is enrolled. Thank them for referrals and ask how the process could be improved.

A smooth referral process doesn’t just support better patient care—it builds lasting relationships and increases volume. It’s a win for patients, partners, and your bottom line.
