Mid-Year Pharmacy Reset: Reassess, Realign, Reignite

June marks the halfway point of the year—a perfect time to pause and reflect on your pharmacy's LTC at Home strategy. At the heart of this reflection should be one question: Are our patients receiving the best possible care? Improved medication adherence, proactive follow-up, and personalized service can make a life-changing difference for patients receiving long-term care at home.

Start by reviewing your patient outcomes. Are there any patterns of delayed refills, missed doses, or increased hospital readmissions? This mid-year reset gives you a chance to assess your pharmacy’s clinical programs. For instance, you might evaluate the effectiveness of your medication reconciliation process, MTM offerings, or care coordination with home health providers.

Next, assess your communication strategies. Are caregivers and patients feeling supported and informed? Do you have tools in place—like text reminders, printed handouts, or follow-up calls—to support adherence?

Once patient care systems are reviewed, shift your attention to business operations. Are your workflows streamlined? Do your technicians feel confident managing LTC@Home tasks? Are your delivery systems on time and well-documented?

Financial growth often follows operational excellence. Realigning team roles, delivery systems, or referral outreach can help optimize both patient care and profitability. Use this month to refresh your goals, reconnect with your mission, and ensure that your pharmacy remains a trusted partner in home-based care.
